Lesson 7.3: Incident and Accident Reporting
Reporting incidents and accidents
All incidents, accidents, and near misses must be reported, regardless of severity.
Accurate reporting helps DIMA Group identify risks, improve safety and meet legal and client requirements.
An incident may include:
• Injuries or illness
• Physical or verbal altercations
• Aggressive or threatening behaviour
• Property damage
• Near misses where harm could have occurred
Your responsibility to report
You must:
• Report incidents as soon as possible
• Ensure reports are accurate and factual
• Include all relevant details
• Cooperate with any investigation
Failure to report incidents or deliberately providing false information may result in disciplinary action.
How to report incidents
Incidents must be reported using C247.
You must:
• Complete the incident report in full
• Submit reports promptly after the incident
• Inform management of serious incidents immediately
In emergencies, your priority is safety. Emergency services should be contacted where required.
Accident reporting
Any accident resulting in injury, no matter how minor, must be recorded.
This includes:
• Minor injuries
• Injuries requiring first aid
• Injuries requiring medical treatment
Accident reporting helps ensure appropriate support and prevents future incidents.
Investigations and follow up
Reported incidents may be investigated to identify causes and improve control measures.
You are expected to:
• Cooperate with investigations
• Provide honest and accurate information
• Follow any updated procedures or instructions
